OT Application Form Modified

Please be advised that modifications have been made to the OT Application Form for General Registration:

1. In question 8 where the applicant answers ‘yes’, this now instructs applicants to ‘go to question 13’
2. In question 24 where the applicant answers ‘no’, this now instructs applicants to ‘go to question 26’

The revised form is available on the website here. It is identified by the footer date stating ‘Effective from: 22 February 2012’.

Applicants are advised to check the footer date of their form before completing an application, to ensure they have the current version.
